About The Position

What You'll Do: Educate and Inform: Guide patients, families, and healthcare professionals through hospice care options and Samaritan’s available services and programs. Customer Service: Provide superior customer service and compassionate support via phone and email. Client Coordination: Handle new patient intakes, coordinate services, and resolve scheduling needs or emergent issues in collaboration with clinical and administrative staff. Relationship Building: Cultivate strong relationships with referral sources, payors, community organizations, and internal teams to ensure seamless care delivery. Administrative Support: Manage backend workflow tasks and maintain accurate documentation using EMR and other systems. Obtain and verify all necessary demographic, clinical, and insurance information. Team Collaboration: Work alongside Customer Service Representatives, Financial Counselor and clinical staff to support and enhance delivery service.

Requirements

  • High school diploma required; associate or bachelor's degree preferred.
  • Minimum of 2 years experience in a medical, hospice, or home care environment.
