About The Position

Responsible for the management of Critical Time Intervention (CTI), a short-term intensive case management program which provides support for individuals with dual diagnoses of mental health conditions and developmental disabilities. Maintain a small case load of clients and provide supervision to CTI case managers, ensuring fidelity to the CTI model. Coordinate with other service providers with in the county.

Requirements

  • Licensed Mental Health Clinician- LMHC-Licensed Mental Health Counselor, Licensed Creative Arts Therapist, Licensed Psychologist or LCSW- Licensed Clinical Social Worker. Prior OMH experience preferred.
  • A strong background in case management in a mental health, social services, or housing program and supervisory experience.
  • Those in process of being licensed and undergoing supervisory hours are also eligible for consideration.
  • Authorized to work in the United States.
  • Excellent oral and written skills
  • Ability to effectively communicate with various socio-economic and culturally diverse backgrounds
  • Good organizational and time management skills
  • Understand and embrace person-centered services
  • Computer literacy

  • Prior OMH experience preferred.

Responsibilities

  • Implement program goals, objectives and strategies in alignment with client needs and relevant methodologies.
  • Implement quality assurance protocols to ensure the delivery of high-quality, evidence-based case management services.
  • Monitor and evaluate program operations, including client outcomes, staff performance, and client satisfaction.
  • Maintain accurate and up-to-date client records, data, and documentation in accordance with organizational policies and funding requirements.
  • Ensure the team delivers personalized, culturally competent, and trauma-informed care to all program recipients.
  • Coordinate in person meetings with clients and complete all assessments and collaboratively develop CTI phase work plans with clients.
  • Monitor client progress and outcomes and adjust service plans as needed to support successful transitions to permanent housing and stabilization of mental health
  • Establish and maintain partnerships with community organizations, housing providers, healthcare facilities, and other stakeholders to enhance service delivery and support client needs.
  • Collaborate and coordinate care with community partners.
  • Advocate for clients at the community and policy levels, working to reduce barriers to services and increase access to resources.
  • Co-facilitate weekly CTI team meetings to review client progress, address challenges, and ensure consistent service delivery.
