The Advanced Heart Failure Therapies Clinic is seeking a Pulmonary Hypertension RN Organ Care Coordinator. This full-time position involves facilitating clinical management and continuity of care for patients with advanced organ failure who require complex medical regimens. The role includes patient assessment, collection and analysis of clinical data, patient education, collaboration with physicians, and participation in research activities. The RN will assess organ failure patients during clinic visits, arrange for routine diagnostic testing, and communicate with clinic and referring physicians regarding patient conditions and needs. The RN will also provide comprehensive teaching and counseling to patients and families, monitor and evaluate clinical problems, and ensure continuity of care as patients transition from hospital to outpatient settings.

  • Facilitates clinical management and continuity of care for patients with advanced organ failure.
  • Assesses organ failure patients during clinic visits and arranges for routine diagnostic testing.
  • Communicates with clinic physicians, referring physicians, and other health professionals regarding patient condition and needs.
  • Provides comprehensive teaching and counseling to patients and families.
  • Identifies and addresses potential deterrents to compliance with medical regime.
  • Monitors, evaluates, and communicates relevant data regarding clinical problems to the interdisciplinary team.
  • Provides teaching and discharge planning in collaboration with hospital nursing staff.
  • Monitors progress of hospitalized organ failure clinic patients.
  • Maintains organized and accessible medical records.
  • Participates in ongoing medical and nursing research activities relative to organ failure patients.
  • Registered Nurse license issued by the state in which the team member practices.
  • Associate's Degree in Nursing.
  • Typically requires 3 years of experience in patient care, including care of patients with advanced organ disease.
  • Cardiac experience and high level of autonomy.
  • Critical thinking skills while triaging patients when taking call.
  • Cardiac experience, ideally ICU experience.
  • Advanced clinical practice experience with end-stage organ disease.
  • Strong interpersonal skills.
  • Ability to work with multidisciplinary teams.
