The position provides support and stabilization services to clients on a crisis residential unit, offering peer and crisis support services, information, education, and support for clients. The role promotes wellness, recovery, and skill building to sustain better health through prevention training for managing stress and problems, making healthy lifestyle choices, recognizing and responding to relapse, managing crisis, and achieving and sustaining recovery. The position assists clients with personal recovery goals, helps clients develop natural support systems, and coaches clients to advocate for themselves within their healthcare systems. It also implements evidence-informed wellness interventions.

Requirements

  • Candidate must be 21 years of age per funding requirements.
  • Must meet Mental Health Professional Status based on Section 140.
  • Bachelor’s Degree in human services related field preferred.
  • Two years relevant experience in human services.
  • Experience participating in their own personal recovery process.

Responsibilities

  • Provides group or individual training/interventions in crisis residential unit, focusing on improvement of the client’s functioning and health-related issues.
  • Assesses level of risk for clients in crisis and assists clients in developing crisis safety plans.
  • Provides wellness consultation.
  • Works in consultation/collaboration with clients, family members, and other providers to implement plans.
  • Empowers clients to self-manage chronic health conditions and solidify wellness goals.
  • Assists clients with finding solutions utilizing motivational interviewing.
  • Assists clients in overcoming barriers to goals, including access to care.
  • Assists with navigating health care systems.
  • Links clients to community-based social support/services and develops collaborative relationships with other departments, services, and community health care agencies.
  • Provides ongoing case management services for assigned service recipients.
  • Conducts intake for recipients entering services, including explaining services and gathering information.
  • Conducts assessment and writes assessment summaries.
  • Assists clients with food preparation and Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL).
