About The Position

Monroe Community Mental Health Authority is a community based mental health service organization dedicated to consumers living in Monroe County. We provide individual health services for people with any type of behavioral health concerns such as substance abuse, emotional issues, and intellectual and developmental disabilities. Our mission is to enrich lives and promote wellness. We live out that mission by being an inviting resource that offers in-house, referral, and emergency services, as well as support within the community for those in need. The shift for this position is Monday, Tuesday, and Friday 7:00am-8:30pm The Crisis Mobile Response Clinician will triage incoming behavioral health/substance use crisis calls and deploy to community settings (i.e., family home, school, shelter, library, etc.) as needed in order to effectively and efficiently manage consumers in crisis. Crisis response may include but not limited to assess, engage, de-escalate, stabilize, safety plan, link and coordinate services, pre-screen, assessments, and hospitalization level of care decisions for Medicaid or indigent consumers of Monroe County based upon state guidelines for hospitalizations. The Crisis Mobile Response Clinician will also be responsible to ride along with law enforcement to assist with de-escalation of a behavioral health/substance use crisis as deemed appropriate. The Clinician will also interface with the community leaders, law enforcement and other providers.

Requirements

• To perform this job successfully, an individual must be able to perform each essential duty satisfactorily and be self-directed in organizing time and tasks.
• Bachelor’s Level (Pay Grade 8: $28.45-$35.57/hr) Preferred: Bachelor’s Degree in Social Work with State of Michigan Licensure (LLBSW, LBSW).
• Acceptable: Bachelor’s Degree in a related Human Services field.
• Acceptable Degrees: Child Development/Family Relations, Occupational Therapy, Community Mental Health, Chemical Dependence Administration, Physical Therapy, Counseling/Guidance, Psychology, Criminal Justice, Recreational Therapy, Rehabilitation Counseling, Social Work, Education, Sociology, Gerontology, Special Education , Health Administration, Speech Pathology , Health Education, etc.
• Master’s Level (Pay Grade 10: $33.46-$41.83/hr) Master’s degree in social work, psychology, or counseling with State of Michigan Licensure.
• Master’s level such as: Licensed Master’s Social Work, License Psychologist, Licensed Professional Counselor, Licensed Marriage and Family Therapist
• The approved licensures for disciplines identified above include full, limited, and temporary limited licenses.
• Valid driver’s license, proof of current automobile insurance and driving record determined by the Authority’s insurance carrier to be eligible for coverage if required as part of positions duties.

Nice To Haves

• Two years previous experience with Mentally Ill or Co-Occurring (MI/Substance Abuse) populations preferred.
• One year experience with children with SED and adults/children with a developmental disability preferred.
• Demonstrated knowledge and skills necessary to provide the appropriate care for the specific clinical needs and ages of the individuals served.
• Demonstrated knowledge of the clinical needs, symptoms, and behaviors of individuals served, as well as understand the principles of growth and development over the life span; have the ability to assess data reflecting each individual’s status; interpret the appropriate information needed to identify each individual’s requirements in relation to his or her age-specific/population specific needs; and to provide the care needed by the appropriate age group - (adolescent, adult and geriatric).
• The following physical and mental abilities are required: ability to work in a complex, detail oriented, deadline and structured environment; frequent lifting or carrying up to 15 pounds, must be able to hear, frequent standing/walking, ability to communicate effectively in person and/or phone, the ability to read, analyze and interpret information, ability to write correspondence and procedures, ability to maintain records, ability to define and solve practical problems, collect data, establish facts and draw valid conclusions, ability to influence others.
• Demonstrated proficiency in Microsoft Outlook, Word and such other applications if/when required.
• Ability to be trained and successfully complete other computer training as required.

Responsibilities

• Demonstrates ability to identify severity of illness by assessing symptom acuity, functional abilities, and risk factors.
• Demonstrates ability to accurately assess psychiatric symptomatology and risk of harm to self or others.
• Demonstrates ability to assess, complete, and document an accurate behavioral health diagnostic impression diagnosis based on the current Diagnostic and Statistical Manual (DSM) and knowledge of corresponding ICD codes, and treatment recommendations. This includes diagnoses related to mental illness (or serious emotional disturbance), intellectual disabilities/cognitive impairments, substance use/abuse/dependence for all ages.
• Demonstrates ability to develop an evaluation of what is causing a crisis situation and develop a plan of safety, which includes alternative resources to hospitalization.
• Demonstrates ability to perform pre-screening for admissions for psychiatric hospitalization.
• Knowledgeable about the special needs when working with people diagnosed with developmental disability, mental illness, and children with serious emotional disturbance.
• Demonstrates knowledge of health insurance benefits.
• Demonstrates knowledge of Duty to Warn, Child Abuse and Neglect, and Elder Abuse reporting laws and the ability to complete mandatory reporting requirements.
• Demonstrates knowledge of eligibility criteria and medical necessity criteria for CMH services, based on current state and federal standards. This includes but is not limited to the current MI Mental Health Code and the current MI Medicaid Provider Manual.
• Demonstrates knowledge of state, federal, and local standards and practices related to service utilization, utilization management, and utilization review.
• Screens all incoming calls, providing crisis service for adults, children, adolescents and families, provides face-to-face assessments consistent with authority standards as needed, schedules admission appointments, or makes referrals to outside agencies or service providers consistent with signed contracts and/or interagency service agreements, including but not limited to, Peace Officers, Jail Administrations and staff, Prosecutor Office, Defense Attorneys, Judges and Probation Officers.
• Coordinates supports/services with the consumer’s identified primary health care provider and upon consent, sends letter to the primary care physician according to agency standards.
• Assesses need for psychiatric consultation authorization.
• Provides individual and group community education by phone and/or by group presentations.
• Works in conjunction with other Access System staff to triage, coordinate the provision of appropriate crisis intervention, emergency service status program placements, follow-up, and after care, and to provide initial and continued stay level of care authorizations.
• Assigns consumers to appropriate Clinical Department of the authority based upon triage assessment and provides level of care authorizations for continuing care consistent with authority standards.
• Maintaining complete and detailed case records and service activity documentation consistent with MDHHS best practice guidelines and standards, authority procedures, and requirements of third-party payer sources.
• Implements interdisciplinary peer review recommendations and decisions on a retrospective and prospective basis.
• Completes assessments/pre-screens, determining appropriateness of emergency psychiatric hospitalization to community hospitals.
• Responds to consumer requests for emergency services via phone or face-to-face contact consistent with authority standards in an effort to stabilize the crisis situation.
• Completes assessments of children and adults to determine appropriateness of emergency psychiatric hospitalization.
• Completes alternative emergency service status placements when assessments determine they are appropriate.
• Performs other related duties as assigned.
• Transportation of consumers when necessary.
• Identifies, operationalizes, and implements, with approval, performance improvement indicators for this program area.
• Actively participates in agency-wide performance improvement activities.
• Maintains individual credentials and participates in education and training necessary to maintain current competency(s) in skill areas essential for implementing job duties and responsibilities.
• Follows oral and written direction completing tasks as assigned by due dates.
• Maintains working knowledge of software applications used in performance of daily duties.
• Complies with and maintains competence with all applicable laws, regulations, standards, and rules governed under Medicare, Medicaid, Michigan Department of Health and Human Services (MDHHS), The Joint Commission, and any applicable funding sources.
• Understands, adheres, and maintains competence with all MCMHA and CMHPSM standards, policies, procedures as they appear in MCMHA’s, Management of Information, Finance-Contract Management, Recipient Rights, Performance Improvement, Administration, Human Resources, Environment and Provision of Care Policies.
• Complies with and maintains competence with the Health Insurance Portability and Accountability Act (HIPAA) regulations as they pertain to privacy and confidentiality.
• Interacts appropriately with managers and peers.
• Performs required duties with an awareness of impact upon the Authority structure as a whole.
• Works as a Team Player within the entire Authority Structure.
