About The Position

The Crisis Engagement Stabilization Specialist will be embedded within the Social Work Department at CHI Lasting Hope. This role focuses on addressing housing crises by engaging directly with individuals and families in distress. Using a strength-based, solution-focused approach, the specialist will conduct supportive conversations to help identify creative, flexible, and cost-effective strategies—often with little to no financial support—to resolve a housing crisis quickly and safely. This position is grounded in empowerment, with an emphasis on respectful, nonjudgmental service regardless of a participant’s perceived needs or barriers. The specialist will also facilitate referrals to appropriate crisis or emergency housing services when immediate resolution is not possible.

Requirements

  • Proven ability to engage with individuals experiencing crisis and build strong, trust-based relationships.
  • Strong organizational skills and attention to detail.
  • Excellent oral and written communication skills.
  • Ability to manage competing demands and respond to high-stress situations calmly and professionally.
  • Ability to work independently with minimal supervision, as well as collaboratively within a team.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and database systems (e.g., HMIS, Clarity).

  • Familiarity with housing resources, crisis resolution strategies, and trauma-informed care preferred.

Responsibilities

  • Conduct comprehensive intake and housing assessments for participants referred by CHI Lasting Hope social workers.
  • Determine participant eligibility for Together’s services and funding support.
  • Use motivational interviewing and solution-focused techniques to explore safe, immediate housing alternatives.
  • Provide services in a non-judgmental, trauma-informed manner, respecting the dignity and autonomy of each participant.
  • Advocate for participants with landlords and property managers to prevent eviction or secure stable housing.
  • Collect required documentation from participants, including identification, income verification, and Social Security cards.
  • Collect documentation from property managers (e.g., lease agreements, rental ledgers, W-9s, deposit or past-due rent forms).
  • Contact utility companies to make pledges and obtain billing statements.
  • Coordinate with the accounting team to request, process, and distribute checks to stabilize the participant’s housing crisis.
  • Develop individualized goal plans in collaboration with participants to support long-term stability.
  • Document all services and financial assistance in the CHI Crisis Engagement program within Clarity.
  • Maintain and update referral and service tracking spreadsheets shared with CHI Lasting Hope staff.
  • Track disbursements and enter check information in the Front Door Excel log as needed.
  • Provide internal and external referrals for additional supportive services.
  • Offer advocacy, information, and guidance to help participants access community resources.
  • Demonstrate knowledge of local services, supports, and housing systems.
  • Respond to community and agency requests in a timely and professional manner.
  • Communicate effectively with CHI Lasting Hope social workers and other community partners.
  • Attend staff, partner, and community meetings as assigned.
  • Foster positive working relationships with local organizations to expand resource access.
  • Maintain complete, accurate, and timely case notes and documentation in both HMIS and internal systems.
  • Prepare outcome reports, summaries, and data related to participant progress, challenges, and success stories.
  • Ensure participant records are confidential, organized, and maintained in compliance with organizational standards.
  • Adhere to facility dress code and uphold a professional demeanor at all times.
  • Perform other duties as assigned.
  • Remain flexible and responsive to evolving needs and service demands.
  • Contribute to a supportive and collaborative team environment.
