Requirements

• QMHP Certification (QMHP-A/QMHP-C/CSAC)
• Bachelor's degree in a relevant field
• Experience in mental health counseling and patient care
• Knowledge of discharge planning and community mental health services
• Valid driver's license and reliable transportation
• Active car insurance coverage
• Ability to obtain an official copy of college transcript

• Experience providing Community Crisis Stabilization services
• Knowledge of community mental health resources

Responsibilities

• Provide crisis stabilization services to adults, adolescents, and children at imminent risk of hospitalization.
• Coordinate the development and timely submission of Individualized Service Plans and required documentation.
• Maintain compliance with DBHDS and Medicaid standards.
• Complete Serious Incident Reports within 24 hours.
• Meet weekly with clinical supervisor to staff and assign/reassign cases.
• Perform client file audits to ensure compliance with Medicaid and DBHDS standards.
• Complete safety planning on day 1 of service initiation.
• Investigate and report Human Rights reportable incidents according to agency policies.
• Adhere to agency policies and complete tasks assigned by Crisis Supervisor and Program Director.
