Crisis Care Coordinators provide crisis stabilization services and linkage to community based services to clients in need of immediate support for a period of one to six months. Individuals are identified by Crisis Services as in need of further stabilization and/or an individual awaiting placement into a long term level of care. Services are provided in the community, at locations such as the person's resident, doctor's clinic, etc.
