About The Position

The Central County Housing Navigator (herein referred to as “Navigator”), is responsible for carrying out the Housing Navigation Services Project, as funded under the Snohomish County Human Services Department. The ideal candidate will be able to manage and provide quality navigational services to an intense workload of homeless and imminently homeless families and individuals as assigned through the Coordinated Entry system. The Navigator will work in partnership with the Operations Director and associated social service providers in meeting the established outcomes of the Project, covering the communities of central Snohomish County: Snohomish, Lake Stevens and Granite Falls.

Requirements

  • BA degree in Human Services or a related field. Related experience can be substituted for educational requirements.
  • Experience in human services program delivery and case management.
  • Excellent interpersonal skills.
  • Excellent oral and written communication skills.
  • Knowledge of available community resources and referral agencies, specific to the Coordinated Entry system.

Responsibilities

  • Complete the Coordinated Entry Assessment for eligible households and individuals, providing quality housing navigation services to literally homeless individuals and families in accordance with Coordinated Entry Policies & Procedures. Provide Rapid Resolution navigation as necessary to individuals and families experiencing homelessness that will not receive housing referrals.
  • Maintain accurate and complete participant files and information electronically in Homeless Management Information System (HMIS) and as applicable in the CRC’s Extended Reach system.
  • Administer the Snohomish County Fenn-Jorstad Self Sufficiency Matrix to all adults served, and enter promptly into HMIS minimally at participant entry and exit. Upload all applicable and necessary participant documentation to HMIS within five days of initial contact.
  • Identifies barriers to housing stability for participants, while partnering with local resource centers in Snohomish County to provide creative rapid rehousing strategies.
  • Knowledge of available resources and available referrals around key aspects of housing stability; i.e. DSHS, financial counseling, food and clothing banks, mental health/substance use treatment, etc.
  • Strong communication and motivational interviewing skills around educating individuals and families about the availability of housing services and eligibility criteria.
  • Provide support to participant’s creation of action plans, establishing attainable and appropriate action plans towards each participant’s housing stability.
  • Attend Coordinated Entry and Navigator meetings as administered by Snohomish County Human Services, actively participating and sharing about system gaps specific to East County.
  • Attend Community Resource Center meetings and work in regular partnership with CRC staff at maintaining a high level of customer service to participants.
  • Must be able to meet clients outside the office on occasion, as well as work occasionally outside regular business hours to meet the needs of clients experiencing homelessness.
  • Other duties as assigned.
