About The Position

The Coverage Coordinator serves as a vital resource within the Case Management Agency (CMA) during periods of leave and transition, ensuring continuity of care for individuals and families. This position manages case assignments, bridges communication among team members, clients, and external agencies, and promotes timely and effective problem-solving. The Coverage Coordinator will oversee a variety of cases, ensuring that client needs are met with professionalism and compassion, while adhering to Medicaid, HCPF, and TRE policies.

Requirements

  • Extensive understanding of Medicaid waivers (IDD, Non-IDD, Children’s), long-term services and supports (LTSS), and community resources.
  • Strong skills in crisis planning, conflict resolution, and interagency collaboration.
  • Excellent verbal and written communication skills, able to navigate sensitive issues diplomatically.
  • High level of emotional intelligence, resilience, and cultural competence.
  • Strong organizational and prioritization skills in time-sensitive situations.
  • Comfort with ambiguity and evolving circumstances.
  • Proficiency in Microsoft 365, cloud-based platforms, CCM, Bridge, and other state systems.
  • Bachelor’s degree. Equivalent experience with LTSS populations, Medicaid Waivers, or working with children with disabilities or a combination of education and experience may be considered in lieu of degree.
  • A reliable vehicle, valid Colorado driver’s license (unless part of an active military family), and proof of insurance with minimum preferred liability coverage of $100,000/$300,000/$100,000

Nice To Haves

  • Previous experience with Medicaid waivers (CHCBS, CES, CLLI, CHRP), developmental disability services, or pediatric medical case management.

Responsibilities

  • Manage case assignments during transitions, ensuring all cases are documented and up-to-date.
  • Act as a liaison between individuals, families, service providers, and CMA leadership during periods of absence.
  • Coordinate and participate in multi-agency meetings to address service disruptions and access barriers.
  • Ensure thorough documentation in Care and Case Management (CCM), Bridge, and other relevant systems.
  • Respond to escalations from clients, guardians, or agencies, providing accurate information and follow-up.
  • Support team members by consulting on challenging cases, offering expertise and guidance.
  • Collaborate with the Supervisor and Manager to identify trends in escalations and recommend improvements.
  • Provide training and support to new team members on case management processes and de-escalation strategies.
  • Monitor service authorization timelines and regulatory compliance for assigned cases.
  • Attend wraparound, DHS, or complex care meetings as needed to support continuity of care.
  • Provide backup coverage for absent team members, particularly during high-need situations.
  • Participate in internal workgroups focused on crisis response, system redesign, or policy adaptations.
  • Utilize TRE tools, such as the Predictive Index, to enhance team dynamics and communication.
  • Engage in strategic planning, quality improvement initiatives, and performance development processes.
  • Adapt to organizational and regulatory changes to maintain service excellence.
  • Represent TRE and the CMA with integrity in all professional communications and interactions.
  • Participate in TRE-led development opportunities and staff engagement activities.
  • Assist with special projects, case reviews, and root cause analysis related to elevated cases.
  • Perform other duties as assigned to support organizational mission and client wellbeing.
