About The Position

The Guardian, will be the Legal Court-appointed Guardian and advocate for clients who have been deemed incompetent by the local probate courts. Legal court responsibilities include the fiduciary acceptance (a legal entity that has the responsibility and power to act on behalf of another person in making decisions in the best interest and providing suitable maintenance when necessary). This is a community based position that requires you to work primarily in the field, with regular travel to conduct home and facility visits and attending court hearings for clients in the Butler/ Warren County area. You will assist in making decisions and following through on decisions made for clients such as living arrangements, medical decisions, quality of life improvements, etc. You will work closely with the team as they use their skillsets to work with an often-difficult population who are incompetent in making independent decisions while respecting their choices as much as possible. In this position you will need to use your experience to seek to understand all the options before making decisions regarding the client’s medical consents, living arrangements, coordination of benefits. You will also be required to regularly come into the office.

Requirements

  • Minimum Associate degree
  • Guardianship certification must be obtained within 18 months of eligibility to sit for certification testing.
  • Must be an Ohio resident.
  • Valid driver’s license and proof of auto insurance as position requires community-based visits/meetings.
  • One year social service experience (including internships-6 months) working with diverse clients for bachelor level candidates.
  • Two years’ experience for candidate with less than bachelor education.
  • Visual/hearing and physical ability sufficient to communicate orally and in writing.
  • Ability to deal effectively with stress.

Nice To Haves

  • acceptable certification such as certified case manager, LPN, Licensed mental health counselor. LSW, Nurse, or Bachelors degree from an accredited institution in related field working with diverse clients including elderly and those with mental health and intellectual disabilities diagnosis preferred.
  • Must have acceptable criminal records check.

Responsibilities

  • Staff Guardians assist in making decisions and following through on decisions made for clients such as living arrangements, medical decisions, quality of life improvements, etc. In this entry level role, Staff Guardian II and III team members will provide assistance.
  • Manage and maintain a caseload including all required documentation in accordance with agency policy and procedure.
  • Make required visits to clients within their home setting (i.e., nursing home, group home, independent living, assisted living, apartment, house)
  • Attend inter disciplinary care conferences
  • Provide oversight of clients plan of care
  • Complete assessments for clients referred to the program
  • Participate in court and legal process pertaining to clients
  • Maintain responsibility for client’s finances and applications for benefits as appropriate (i.e. work in coordination with Guardian of Estate, payee, trust officers, etc.)
  • Participate in 24 hour on call rotation
  • Perform other related duties as assigned within the scope of this position
  • Participate in staff organizational meetings, attend conferences, and trainings
  • Maintain professional attitude and represent company in a positive manner at all times
  • Participate in PQI as requested by Manager

  • Community First will provide agency equipment such as laptop and telephone. Employee agrees that equipment will not be used by anyone other than the employee and only for business-related work.
