About The Position

This professional position is responsible for providing case management to individuals meeting criteria for developmental delays, mental health, and/or with intellectual disabilities. Responsible for monitoring, linking, and evaluating services. Link to other services and make referrals as needed. Provide regular direct individual contact and provide face-to-face case management services. Meet productivity standards as set for the program. Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or family. Enter necessary information and maintain information in computerized database, state computer systems, and in charts in accordance with state, federal and licensing regulations and Agency policy. Obtain and update information within required timeframes. Provide crisis intervention services as needed. Work in cooperation with and consult with Agency staff, supervisors, other providers, and community resources as needed. Link individuals to transportation services or transport individuals to appointments as needed. Attend meetings, staffings, and trainings as needed. At times, you may provide coverage in Bedford County. Perform other related duties as assigned.

Requirements

  • Six months of experience as a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • A bachelor's degree with a social welfare major; or
  • A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.
  • You must pass a background investigation.
  • This position falls under the provisions of the Older Adult Protective Services Act.
  • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
  • This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
  • PA residency requirement is currently waived for this title.
  • You must be able to perform essential job functions.
