If this sounds rewarding to you, apply today to join our dedicated team! In this position you will develop plans for consumers with Intellectual Disabilities following the intake process. Develops treatment plans and enrollment into Behavioral Health/Intellectual Disabilities/Early Intervention/Autism programs as appropriate. Develops Individual Support Plans (ISP) with detailed goals and locates providers as detailed on the ISP. Maintains files, records and other documentation for the continuity of care in a timely and organized manner. Develops life management plans and acts as an advocate for the consumer to ensure appropriate treatment as approved. Documents contacts with consumer, family, service providers. Duties are not limited to desk work and may include frequent standing, climbing steps and walking distances. Must be able to climb flights of steps in homes or apartment buildings where no elevators are present. Manages multiple tasks, paying close attention to details under a constantly changing work environment which includes completion of paperwork and meeting strict deadlines. Completion of daily deadlines on paperwork and work assignments.
