Prepare an individualized treatment plan to include goals, interventions, and criteria for achievement. Evaluate client response to treatment and modifies treatment plan as needed. Conduct individual and family counseling sessions with clients. Document counseling services in medical record that is audited by a supervisor on a regular basis to include clinical service notes, progress summaries, treatment plans, assessments, and other needed documentation to meet insurance requirements. Maintain regular communication verbally or in written form with client family members and other medical professionals on an as needed basis. Provide counseling services consistent with the established ethical standards of an LPC, LMFT, or LISW-CP in South Carolina. Perform other related duties as directed or assigned.
