About The Position

The Counselor Social Worker provides evidence-based individual, group, family, and educational services, including assessment and psychotherapy. This role involves documenting clinical and consultative activities in the patient's electronic medical record and participating as a member of a multi-disciplinary treatment team to ensure appropriate patient treatment. The Counselor Social Worker also works to develop and promote the department's programs and objectives.

Requirements

  • Licensed Clinical Professional Counselor (LCPC) or Licensed Clinical Social Worker (LCSW) in the State of Idaho
  • One to three years of experience in a mental health setting required

Nice To Haves

  • Experience in multiple therapy techniques preferred

Responsibilities

  • Conduct evaluation of patients and collaborate with healthcare team members to support the emotional, mental, environmental, bio-psychosocial, and spiritual needs of the patient and/or their family/caregivers, as applicable.
  • Clearly document at each visit objectives, measurable goals and progress toward goals and identify the expectations of counseling with patients.
  • Recognize when a patient has achieved maximum benefit from therapy and professionally terminate care when the patient is not progressing toward goals despite any needed treatment plan modifications or when the patient has reached their goals.
  • Counsel patients and families regarding recommended levels of care, community resources and financial arrangements, as the program defines.
  • Provide interventions including, but not limited to, individual or family counseling, referral to internal and external community resources, crisis intervention, grief counseling, supportive therapy, substance abuse referral, and support groups.
  • Complete all patient care documentation within the standards of the Trinity Health Timely Documentation policy.
  • Participate in team meetings.
  • Mentor students during internships, as applicable.
