About The Position

Join our team as a Counselor II with strong potential for permanent placement. The goal is to transition into a permanent full-time Union position after completing 3 months of temporary employment. We’re seeking a motivated individual who is dependable, adaptable, and interested in building a long-term career with us. Summary: This position will provide eligible children and their families enrolled within the Family Based department community home-based intensive Family (2x a wk), Group (facilitate parent group) and Individual (as needed to prepare for family sessions) therapeutic and case management services under the guidelines and procedures of the program.

Requirements

  • Master's degree or equivalent
  • three to five years related experience and/or training; or equivalent combination of education and experience

Responsibilities

  • The ability to travel to homes, schools, and community locations to provide services.
  • Provide a minimum of family and/or individual therapy weekly for each assigned family while providing ongoing case management, crisis and/or social intervention.
  • Demonstrate the ability to produce a minimum of 18 hours of billable units which includes 10 hours of individual and/or family therapy and 12 hours of team therapeutic/case management services on a weekly basis
  • Demonstrate the ability to work with team partner 60% of the time for family sessions and crisis intervention.
  • Demonstrate the ability to develop and maintains complete clinical records which include assessments, assessment tools (i.e. genogram, eco-gram, and structural map, Modified Family Assessment tool, screening tools) other clinical and administrative documents according to program standards for timeliness. Ensures charts adhere to established chart sequence, date order, and that all documents are present and completed in a timely manner.
  • Timely submission of clinical developed case documentation including assessments (30 days), progress notes (24 hrs), treatment plans (25-30 days), case conferences (with CBH i.e. high risk every 30 days, low risk 30 days and 120 days| video and written presentation during staff meetings and FBS offsite trainings)and discharge summaries (24 hrs)
  • Preform regularly checks of client files to ensure that content, signatures, credentials, dates, times, service types and legibility meet standards.
  • Identifies community resources and facilitates linkages with psychiatric, social, vocational, medical, housing, recreational, forensic, and drug/alcohol systems for the coordination of services to meet clients’ need.
  • No less than 25% of weekly work time in the office completing paperwork, attending bi-weekly staff meetings and clinical rounds, weekly supervision and monthly case conferences as scheduled, in addition to monthly offsite training.
  • Ability to provide a thorough review of current caseload including a progress report and proactive action plans to address areas of concerns.
  • Communicate needs immediately and respond to a communication from the program within 30 minutes of the outreach.
