About The Position

As a Counselor II, you will provide guidance, support, and therapeutic interventions that help individuals navigate life’s challenges and build greater independence. Working under the direction of the Assistant Director of Clinical Services, you will deliver comprehensive rehabilitative services in natural environments, ensuring high standards of clinical quality and regulatory compliance. You will play a key role in developing individualized treatment plans, supporting direct care staff, and collaborating with families, state case managers, and interdisciplinary teams to promote positive outcomes.

Requirements

  • Bachelor’s degree (required)
  • 1–3 years of related experience in rehabilitation, behavioral health, or human services
  • Valid standard driver’s license (Class C or D)
  • Strong interpersonal, organizational, and communication skills
  • Ability to work independently while thriving as part of a multidisciplinary team

Responsibilities

  • Deliver a continuum of neurorehabilitative services in community settings
  • Assist with intake and admissions processes
  • Conduct assessments related to rehabilitation, skill development, behavior, life goals, and quality of life
  • Support the development and implementation of individualized rehabilitative and behavioral care plans
  • Participate in the Individual Support Plan (ISP) process
  • Provide weekly on-site oversight of rehabilitation and plan implementation
  • Serve as a clinical consultant to assigned residential programs
  • Collaborate with administrative, clinical, residential, and external providers (e.g., day programs, mental health professionals)
  • Maintain strong working relationships with families, guardians, and state case managers
  • Participate in staff meetings and interdisciplinary team discussions
  • Provide staff and caregiver training, coaching, and on-site technical assistance
  • Support continuous staff development initiatives
  • Assist leadership in performance improvement and systems development efforts
  • Maintain compliance with Department of Developmental Services (DDS) regulations
  • Complete regulatory documentation, mandated reports, and maintain confidential records
