About The Position

The Counselor Assistant assists in the care of residents and provides support with the daily operation of the facility. The counselor assistant provides person-centered, trauma-informed, care coordination and case management to residents.

Requirements

  • Education: Associates preferred. High School diploma or equivalent required.
  • Credentials: CASAC-T preferred, Valid NY State Driver License and access to a reliable vehicle
  • Experience: Two years of prior experience in substance abuse or related human service field preferred, As per Justice Center recommendations, all counselor assistants must be 21 years of age or older.
  • Relevant years of experience and education will be considered
  • Adheres to all applicable federal and state laws and regulations including, but not limited to, those governing confidentiality, privacy, program, billing and documentation standards. All duties must be performed in accordance with CCDR’s corporate compliance and ethics program.
  • Ability to prioritize assignments, plan, and complete work projects with minimal direction
  • Exceptional time management skills and ability to effectively and meet deadlines
  • Excellent verbal/written skills
  • Previous experience working with diverse populations, including economic diversity.
  • Possession a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier.
  • Continuous use of a reliable, registered and insured vehicle.
  • Ability to work in the following software programs: Windows, Microsoft Office, and multiple online software platforms as required by funders and partner organizations
  • Ability to maintain confidentiality and handle confidential information with discretion

Responsibilities

  • Gets involved in resident’s daily activities, observes behavior and ensures the maintenance of appropriate conduct.
  • Completes comprehensive service plan every 30 days in collaboration with client.
  • Provides for the safety and integrity of the facility and its residents at all times.
  • Ensures all records are locked and medications are stored and monitored according to program policy.
  • Ensures the adherence of confidentiality regulations.
  • Provides referral and care coordination services to residents and communicates with resident’s appropriate service providers.
  • Provides psychoeducation and training to residents in living skills.
  • Communicates effectively and works cooperatively with residents, staff, and community professionals and documents according to reporting and recordkeeping policies.
  • Participates in the development and maintenance of the therapeutic community.
  • Provides basic crisis management as needed and addresses concerns with appropriate clinical staff members and external treatment providers as appropriate.
  • Transports residents in agency vehicles to program related activities.
  • Uses the daily log and progress notes to communicate information gathered during assigned shifts.
  • Provides screening, intake, and orientation services to residents.
  • Co-facilitates groups with CASAC level or higher, as assigned by the Clinical Supervisor.
  • Meets one on one with a caseload of residents for provision of case management services and prepares progress notes at least once per week reflecting on the goals and objectives of the resident’s service plan.
  • Provides comprehensive case management services and complete access to community services for legal, mental health, social services, vocational assessments, and misc. needs.
  • Ensures compliant recordkeeping.
  • Participates in supervision sessions actively and professional development/ training.
  • Adheres to Agency policies and procedures
  • Completes other duties as assigned
