Requirements

REQUIRED: Bachelor's Degree in a Human Services or related field

Responsibilities

Assist individuals in establishing necessary connections with available supports in the community that will facilitate initiation and on-going engagement in the recovery process.
Assists individuals in developing and solidifying long-term recovery supports, and providing services before, during, and following an individual’s engagement in the treatment continuum.
Provides opportunities for individuals to direct their own recovery plan and supports, build self-worth, wellness, empowerment, and self-advocacy.
Work with the individual’s family, service and treatment providers, other programs, and community supports to assist in achievement of these goals.
Initiates and develops treatment planning goals through contact with clients, family, colleagues and other staff.
Performs individual and process group therapy sessions under the direction of the treatment supervisor including preparing materials for sessions.
Manages client files.
Communicates with referral sources and company representatives.
Attends treatment team meetings with psychiatrist and staff.
Participates in community meetings with clients.
Completes family sessions as necessary.
Prepares utilization reviews with appropriate funding sources as needed
Maintains client caseload daily.
Other duties as assigned
