In accordance with RNP’s Mission Statement, Philosophy of Client Care, and Code of Ethics, as stated in the Personnel Polices and under the supervision of the Department Director or Clinical Coordinator, is responsible for admissions at Prospect House Shelter, as well as entering data into the HMIS, DDAP, and RNP database.

  • Screens all potential residents and makes referrals as needed.
  • Assesses and records medical, psychological and social factors contributing to client’s situation and evaluates these and client’s capacities.
  • Completes intake assessment on all new admissions, formulates client’s clinical chart.
  • Enters timely data into HMIS, DDAP and RNP database. Keeps databases up-to-date with admission and discharge information.
  • Compiles daily client census and bed sheet.
  • Prepares and maintains Department of Social Service reports.
  • Participate in weekly staff meetings.
  • Monitor medications
  • The ability to make appropriate referrals at Intake
  • Front desk coverage
  • Uphold confidentiality regulations
  • The ability to coordinate admission with the CAN
  • Knowledge of best practices providing services for individuals with mental health and substance use disorder such as client centered treatment care, motivational Interviewing and trauma informed care.
  • Assumes all other appropriate duties as deemed necessary by the supervisor.
  • Knowledge of the physical, psychological, social and economic effects of homelessness, substance abuse and mental illness.
  • Knowledge of community resources.
  • Considerable knowledge of federal and states laws and regulations regarding client confidentiality.
  • Ability to establish and maintain cooperative professional relationships.
  • Excellent communication skills.
  • Bachelor’s degree in behavioral science and two (2) years’ experience in case management preferred, or equivalent combination of education and experience.
  • Valid Connecticut Driver’s license
  • Co-occurring competencies and or credential are preferred.
  • Incumbents may be required to secure professional certification and/or license.
