Responsible for individualized client services provided in a community/residential setting. Duties include assessing, planning, implementing, monitoring, and evaluating client behaviors and outcomes while ensuring appropriate level/type of services are provided. Position Description: With close supervision; conducts interviews and administers assessments and screening instruments Maintain caseload and productivity by ensuring regular contact with clients to monitor and assess progress Write service plans, review progress and determine updates needed in collaboration with client Utilize core correctional practices while assisting clients in effective community re-integration Deliveron-going services to clients which may include crisis intervention, advocacy, medication monitoring, transportation assistance, financial budgeting,housing assistance,and referral and linkage to community resources as needed Ensure compliance with applicable standards and agency policies Document all clinical activities to meet specific program,funder and regulatory criteria timely and with accuracy Participate in team meetings and assist in making recommendations for client status changes Lead educational groups and/or facilitate group curriculum May require transportation of clients Adhere to professional standards as outlined by protocols,rules, and regulations Other duties as assigned
