The Licensed Practical Nurse works within the healthcare team and provides focused community and clinic health services directed towards the prevention of disease, the promotion of health, improving clients self-care, using professional judgment, screening, contributing to care planning, intervention, and provides health information/education to patients. Licensed Practical Nurse MSR (LPN) in Corrections Health is primarily tasked with the administration of MSR medications (Subxone, Subutex) to incarcerated patients under supervision of a Community Health Nurse (CHN) and supervisor. The MSR LPN also will assist with reviewing MSR referrals, and refer to a nurse or providers as appropriate. This recruitment is used to fill on-call, temporary/limited duration, and regular status positions. This recruitment is for the Corrections Health LPN team as well as the Medication Supported Recovery (MSR), Licensed Practical Nurse positions on the Transition Services Program. In the Licensed Community Practical Nurse role you will: Utilizing electronic reports and review provider schedules to proactively assess needs for prevention screening and communicating via EHR. Pro-actively performs pre-visit activities (e.g. scrubbing and huddling) for client visits as needed, As well as preparing patients for exams, and procedures, including equipment set-up and specimen collection. Working with providers and teams to monitor chronic disease and prevention reports and initiates follow-up with clients or ensures appropriate referrals as needed. Following up on specialty referral reports, clinical treatments, prescriptions, medication cancellations, etc. Documenting clearly in the patient chart, using the SOAP method, if indicated, and EPIC "dot-phrases" and "smart phrases" as deemed appropriate per patient encounter. Supporting back office clinic flow by direct provider support as needed including rooming patients, ordering labs as directed. Providing after visit instructions based on nursing experience and understanding of client needs; makes follow-up appointments, if necessary. Administering medical treatments such as cleaning and dressing wounds, ear irrigations, suture removal, catheterizations, fluoride treatments, etc. Performs diagnostic tests such as labs, phlebotomy, EKG, etc. Work with RN and PCP to contribute to the plan of care. Completing documentation and charting as required by policy. Troubleshooting equipment issues and reporting necessary repairs as needed. Completing assigned Quality Control/Assurance activities.
