About The Position

The position involves maintaining an active caseload of up to 20 – 25 individuals referred for START Services, providing systemic consultation based on assessment and outreach findings. This includes conducting regular outreach visits to individuals’ homes and vocational settings to complete evaluations and assessments, assisting with referrals for consultation and treatment, and ensuring the coordination of support meetings and crisis plan development. The role also requires developing comprehensive service evaluations and intervention plans, participating in meetings with START leadership, and providing on-call support as part of the START team. Documentation and data entry into the START Information Reporting System are also essential responsibilities.

Requirements

  • Master’s degree in Mental Health, Psychology, Counseling, Social Work or other Human Service field.
  • At least two (2) years of experience working with the I/DD population and/or behavioral health needs.
  • Successful completion of START Coordinator Certification training within 12 months of appointment.
  • Ability to provide on-call coverage for crisis services on a regularly recurring basis.
  • Ability to travel regularly across assigned region(s) for assessments and evaluations.
  • Ability to meet essential physical demands of the position.
  • Exceptional interpersonal and problem-solving skills.
  • Independent and organized with strong communication and writing skills.

Nice To Haves

  • Experience working with individuals with I/DD with co-occurring diagnoses and complex behavioral needs.
  • Experience working with people and their families and/or social service providers.
  • Verbal and/or written fluency in Spanish highly preferred.

Responsibilities

  • Maintain an active caseload of up to 20 – 25 individuals referred for START Services.
  • Conduct regular outreach visits to individuals’ homes and vocational settings for evaluations and assessments.
  • Assist with referrals for consultation and treatment as needed.
  • Ensure coordination of support meetings and crisis plan development.
  • Develop Comprehensive Service Evaluations and intervention plans.
  • Participate in recurring consultation and staff meetings with START leadership.
  • Provide on-call support on a regularly scheduled basis.
  • Complete and maintain all applicable documentation and records.
  • Accurately enter required data into START Information Reporting System.
  • Conduct regular outreach with community partners.
  • Develop materials and present at Clinical Education Team Meetings.
  • Participate in the Center for START Services National Network.
  • Complete necessary trainings to ensure compliance with YAI policies and regulatory requirements.
