As a member of the Opioid Response team, the Opioid response Linkage Navigator role will support CrescentCare’s community based navigation and opioid response efforts. The Prevention Department at CrescentCare supports several opioid response programs, including the New Orleans Syringe Access Program and the new Opioid response Mobile Unit. The Opioid response teams work to link key populations to opioid response services and care, and coordinates with community partners to implement opioid response programs across the New Orleans Region. The Navigator will conduct counseling and testing sessions, serve as navigation and linkage for patients accessing the mobile unit, support CrescentCare’s opioid response programs and community of practice, and support adherence/retention of clients in HIV/STI/Hepatitis C(HCV) services through linkage and referrals into other Prevention programming and agency wide services. Work with the Prevention Director, Prevention Linkage team, and other agency departments and partners, including medical providers, to actively test, link, and navigate clients to services including medical care and essential support services. Assist in compliance with funder requirements and attainment of program and outreach deliverables. Assist supervisor and managers with meeting program area requirements, including data management, analysis and reporting, staff and staffing needs, supplies, and all quality assurance (QA) measures. Ensure all reporting and evaluation mechanisms are complied with. Have a working knowledge of opioid response policy.
