About The Position

Clinical Practice Coordinators are an extension of clinical expertise and collaborate with business operations to further grow and develop frontline clinical staff. This position is responsible to promote and establish evidence-based nursing care standards and efficiencies to ensure clinical staff are practicing at the highest level of scope or training. The clinical practice coordinator facilitates education and competency oversight of clinical initiatives. This position is accountable for promoting safe, and high quality patient care by adhering to system policies, procedures, protocols, and standing orders through innovative objectives/strategies for achieving clinical outcomes. This position serves the Home Care division including Home Health, Hospice and Outpatient Palliative Care.

Requirements

• Bachelor's in Nursing.
• RN in state of Illinois or Michigan.
• 2 years of healthcare experience.
• Knowledgeable on Electronic Medical Record.
• Excellent interpersonal and communication skills.
• Solid computer skills, including proficiency with Microsoft software.
• Strong analytical and problem solving skills, with the ability to be detail oriented.

Nice To Haves

• Certification in area of specialty.
• Epic knowledge.
