Responsible for data collection and abstraction for various quality/safety/improvement projects and publically reported measures. Performs a wide range of duties involving the collection, abstraction, and recording of clinical patient data following department guidelines and standards. Reviews cases as needed based on current performance improvement or abstraction focus. Relays information to clinicians or improvement teams based on data collection/abstraction in order to improve performance. Is a member of performance improvement or service line teams as appropriate. The position focuses on disease specific abstraction, analysis and improvement working closely with the Stroke Coordinator and other Quality, Safety and Regulatory Team members.

Requirements

  • Bachelor’s degree in Health Care Related Clinical field.
  • Two years of experience working with medical records clinical abstraction and performance improvement in healthcare.
  • Excellent oral and written communication skills and teamwork skills.
  • Ability to utilize a variety of computerized software applications such as word processing, spreadsheets, and databases required.
  • Ability to work independently.
  • Must have proven organization skills to meet established internal and external deadlines.
  • Must be able to exercise sound clinical judgment in the interpretation and documentation of medical and nursing care.
  • Ability to work with all levels of the organization and to actively participate on improvement teams.

Nice To Haves

  • EMR experience.
  • 2 years acute care clinical experience a significant plus.

Responsibilities

  • Performs concurrent and retrospective medical record review to: a. Meet requirements for abstraction put forth by government, accreditation, and insurer requirements. b. Identify quality of care concerns and performance improvement opportunities as identified by specific UMMMC indicators.
  • Identifies and retrieves data (per specific project requirement) from electronic record or other data source based on abstraction needs.
  • Coordinates data collection for abstraction along with any outside vendors; completes appropriate forms and enters data into electronic data base(s) and may submit to external sources.
  • Interacts with healthcare stakeholders to share information regarding performance improvement initiatives.
  • As necessary, make outgoing post discharge phone calls to selected patients to obtain information from patients.
  • Facilitates and/or participates in a variety of task forces and quality improvement meetings. Reports on data for internal sources.
  • Works collaboratively with Medical Records Department, and Service Lines to identify areas for performance improvement around documentation, coding, quality, and clarification of externally and publically reported data.
  • Remains current regarding changes in hospital and governmental/external agency regulations as related to pertinent data collection.
  • Works collaboratively with Information Technology in the development of tools/reports for electronic abstraction of key fields in the EMR.
  • Reviews protocol for compliance with established databases, laws and regulations or organizational policies and procedures such as TJC, UHC and CMS.
