About The Position
The Case Manager is responsible for the delivery of patient care services through coordination, implementation, and supervision of patients. With a Quality, Customer First, and Compassionate approach, the Case Manager will identify clients that will benefit from care coordination, work with agencies and service providers to facilitate care coordination and information sharing, and provide case management to assigned patients. The role involves collaboration with providers, clinical staff, and community resources to promote access to appropriate care, ensuring patient visits are efficient and effective. The Case Manager will also develop client-focused case management plans, document ongoing activities in the Electronic Health Record (EHR), and assist patients with various needs including scheduling appointments and transportation.
