About The Position

The Health and Welfare Council of Long Island (HWCLI) is a private, not for profit, health and human services planning, research/public education and advocacy organization that serves as the umbrella for public and non-profit agencies serving Long Island's poor and vulnerable individuals and families. The Health Equity Alliance of Long Island (HEALI) is Long Island’s Social Care Network (SCN) of community-based organizations (CBOs) and healthcare providers building a person centric model that integrates healthcare, social care, and behavioral health care. HWCLI is the lead entity for the HEALI Social Care Network. The SCN brings together social service and health care providers from across Nassau and Suffolk counties to provide enhanced healthcare equity through improvements in need identification, care coordination, integration, and provision of tailored funding. JOB ANNOUNCEMENT: Coordination Center Associate, Health Equity Alliance of Long Island HWCLI seeks an experienced, energetic, passionate, and socially conscious individual to support the HWCLI and HEALI mission. This position reports to Community Health Worker Manager.

Requirements

  • High school diplomas required
  • Experience in healthcare coordination, care navigation, case management support, or administrative support roles
  • Experience handling inbound calls and supporting members or clients in a service-oriented role preferred
  • Familiarity with social service programs, community-based organizations, and referral networks preferred
  • Strong communication skills, including active listening and the ability to adapt communication style to different audiences
  • Ability to solve problems and identify solutions in a fast-paced, evolving environment
  • Strong organizational skills, multitasking abilities and attention to detail
  • Strong interpersonal skills with the ability to tailor style to match audience
  • Demonstrated ability to work independently while also collaborating effectively within an interdisciplinary team, managing multiple priorities

  • Proficiency in Microsoft Office (Outlook, Word, Excel)

Responsibilities

  • Receive, review, and distribute all incoming referrals to HEALI, assigning referrals to the internal team or network partners as appropriate
  • Track referral activity and ensure timely follow-up by contracted partners in accordance with network standards
  • Review and route inbound member self-screens and screens ingested from off-platform partners
  • Monitor the HEALI phone line, respond to member inquiries, and assist in addressing member needs and barriers to care
  • Serve as a liaison between care teams and community-based partners to support coordination of care, particularly when members are at risk of disengagement or delayed services, including follow up on provider attestations
  • Facilitate referrals to appropriate programs, services, and resources based on member needs, eligibility, and service availability
  • Produce and maintain required reports related to referrals, member engagement, outcomes, and operational performance
  • Assist with providing select SCN services, working closely with the internal Community Health Workers and Finance, that require ordering, delivery, or scheduling of services through third party vendors
  • Assist with adding Level 1 services provided by Long Island community Based organizations to accept and send referrals to/from HEALI
  • Identify workflow gaps and operational challenges and provide feedback to support continuous improvement
  • Provide operational and administrative support to the navigation and care delivery team as needed
  • Support additional programs and initiatives as assigned
