Jewish Family & Career Services (JF&CS) is seeking a Contract Case Manager to provide case management services to low-income individuals of all ages at risk of homelessness. The Case Manager will assess client needs, develop care plans, coordinate services, and ensure compliance with funding and agency requirements. This role requires experience with HMIS (Homeless Management Information System), strong collaboration skills, and knowledge of community resources.
