About The Position

We are looking for an experienced California licensed vocational nurse ("LVN") to work on-call and/or continuous care shifts. The LVN works under the direction of a Registered Nurse and provides nursing and palliative care to assigned hospice patients. The On-Call / Continuous Care Hospice LVN provides intermittent visits and/or shifts of skilled nursing care, performs prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary caregivers, and participates in the care plan development and implementation with other members of the hospice interdisciplinary team. Must be able to utilize computers and be comfortable with EMR. Must possess a sympathetic attitude toward caring for hospice patients and families and must have positive communication skills interaction with other members of the hospice team. On-Call and Continuous Care shifts are frequently after hours and on weekends. Requirements

  • Graduate of an accredited school of vocational/practical nursing.
  • Current LVN license within the State.
  • At least one-year experience as LVN/LPN.
  • Valid driver's license, and auto insurance, reliable/well maintained transportation.
  • Must be able to utilize computers and be comfortable with EMR.
  • Must possess a sympathetic attitude toward caring for hospice patients and families and must have positive communication skills interaction with other members of the hospice team.
  • Must be willing to drive to different areas

Nice To Haves

  • Certification in Hospice and Palliative Vocational/Practical Nursing preferred.
