About The Position

The Continuing Care Coordinator at Helio Health plays a crucial role in providing rehabilitation services to patients dealing with substance use and mental health disorders. This full-time position is part of the Syracuse Inpatient Program, where the coordinator will maintain a caseload of patients and perform essential counseling responsibilities to support their recovery journey.

Requirements

• Associate's Degree in a human service field with minimum of CASAC-T required, or a Bachelor's Degree in a human service field with demonstrated steady progress towards QHP status.
• One year counseling experience preferred.
• Knowledge of addiction/recovery, behavioral health, self-help group process and principles of counseling.
• Proficiency with basic computer applications.

Responsibilities

• Maintain a case load of patients seeking treatment from the effects of substance use and/or mental health disorders.
• Perform essential counseling responsibilities including screening, intake and orientation, assessment, evaluation and intervention, treatment planning, referrals, crisis intervention, counseling, patient education, case management, reporting and record keeping.
• Participate in program, administrative and clinical committees as assigned.
• Relate effectively with staff, clients, and community while upholding confidentiality requirements.
