Position Description The Registered Nurse (RN) is responsible for providing general nursing care to our patients through collaboration with Physicians and other support staff of the Center. The Registered Nurse will promote and strive to restore a patient's health through education, advocacy, and quality delivery of care. The RN is a key member of a collaborative care team, employing a whole-person care approach to actively engage patients in managing their health. This role requires exceptional teamwork, adaptability, and strong communication skills to address the dynamic and evolving needs of patients. Reporting to the Health Center Practice Manager, the RN is integral to aligning care delivery with Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) standards, ensuring high-quality, patient-focused outcomes through coordinated efforts and innovative problem-solving.
