About The Position

The successful candidate will provide part time pharmaceutical services to long-term and assisted living care residents in a manner that maximizes quality and patient safety. The clinical pharmacist will manage appropriate drug therapy and review of patient medical charts in accordance with federal and state regulations and company policy. The clinical pharmacist travels from home to service long-term and assisted living care facilities in Virginia. Currently the specific areas include Martinsville, Charlottesville, Warrenton, and Orange, Virginia. The clinical pharmacist will be provided a laptop computer, printer, office supplies, and travel reimbursement. New grads encouraged to apply.

Requirements

  • Works as a pharmacy subject matter expert
  • Develops and presents pharmacy reporting to various internal and external audiences including client-specific pharmacy experience/trend reporting
  • Works with pharmacy sales to identify new business opportunities
  • Knows, understands, and communicates pharmacy-related marketplace topics and issues for unique/varied audiences
  • The clinical pharmacist must have excellent interpersonal, verbal, and written communication skills, as well as excellent analytical, problem solving, and time management skills.
  • PharmD
  • Must be licensed or will be licensed in the state of Virginia.
  • Registered Pharmacist in state of practice

Nice To Haves

  • Board-Certified Geriatric Pharmacist preferred, but not required.
  • Long-term care experience preferred, but not required
  • 1 year of long-term care pharmacy practice a plus
  • Willingness to travel up to 80%25

Responsibilities

  • Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability
  • Makes recommendations for therapy changes to the physician and facility staff as indicated and appropriate
  • Reviews the drug regimen and clinical indicators of each resident as assigned at least monthly and reports any irregularities to the Director of Nursing, Administrator and the attending physicians with a written report of findings and recommendations
  • Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members
  • Knowledge of pharmacy laws and federal and state regulations concerning long-term care
  • Ability to work cooperatively as a member of the healthcare team
  • Knowledge of pharmaceutical care practices for long-term care residents
  • Completes monthly or more frequent drug regimen reviews, medication room/cart inspections, medication-pass observations, and educational programs
