Consultant - Community Health and Development supports Carilion's hallmarks of service excellence, innovation, and quality through leadership in program design, execution, and evaluation. This position supports community-based clinical and wellness initiatives, guides departmental strategy, and serves as a liaison to community partners to improve community health and address identified needs across Carilion's service area.

  • Facilitates the community health improvement process, including Community Health Needs Assessments (CHNAs), internal and external health improvement planning, program implementation, and continuous quality improvement.
  • Leads the design and evaluation of evidence-based and outcome-driven interventions, collaborating with internal and external partners to maximize population level impact.
  • Plans and implements evidence-based health education programs and practices to influence health behavior change among underserved audiences.
  • Fosters a collaborative team environment grounded in accountability, outcomes, innovation, and service excellence while championing Carilion's mission, vision, values, and commitment to community health.
  • Represents Carilion on coalitions, advisory boards, and cross-sector initiatives aligned with CHNA findings and public health goals.
  • Aligns community-based efforts with clinical operations and system-wide community health improvement objectives.
  • Applies data, research, and evaluation tools to inform decision-making and demonstrate measurable impact.
  • Facilitates program outcomes and KPI reporting.
  • Creates and maintains dashboards and reports focused on health outcomes, clinical indicators, and return on investment.
  • Assists in the development of business plans, grant applications and reports, and community benefit reporting.
  • Engages patients, families, and community members to gather input and improve programming.
  • Develops and manages program budgets; identifies cost-effective strategies to advance goals.
  • Consistently delivers the best in customer service and patient care.
  • Champions Carilion's mission, vision, and values; complies with the policies and procedures, ethical standards and code of conduct set forth by the company.
  • Bachelor's degree or equivalent with extensive experience required; Master's degree preferred.
  • 5 or more years experience managing projects pertaining to clinical strategies, community health improvement, business, or leadership.
  • Excellent oral and written communications skills.
  • Knowledge of business practices.
  • Positive attitude and self-motivated.
  • Ability to work with minimal direction.
  • Ability to lead and adapt to change.
  • Highly skilled in use of personal computers.
