Located off the beautiful Oregon Coast, Adventist Health Tillamook has been one of the area's leading healthcare providers since 1950. We are comprised of a 25-bed critical access medical center, rural health and urgent care medical offices, home care services, and a vast scope of services located throughout Tillamook County and the surrounding areas. The allure of Tillamook's community is complemented by access to outdoor adventures and the scenic Oregon Coast. As a Constant Observer, you will provide direct behavioral support to patients at risk of harm to self or others due to mental illness or inability to care for themselves. Under the direction and supervision of a registered nurse (RN) and leadership, you will maintain proper supervision of patients, ensuring they are within line of sight, acting as a room sitter or providing one-on-one support. Your role will involve identifying behaviors and statements that require intervention by psychiatric staff, ensuring the safety and well-being of patients in a critical care environment. You will monitor patients on involuntary holds initiated by law enforcement or court order, as well as those deemed at risk of suicide or self-harm. This includes monitoring patients who need fall protection, are in a state of confusion or agitation, and are at risk of self-harm. You will assist patients with activities of daily living within your scope of practice and report any changes in behavior to the RN or provider. Following safety protocols and infection control policies is essential, and you may be assigned other job-related duties as needed.
