The Connect2Thrive Partner Integration Sr. Manager is responsible for managing and advancing Connect2Thrive’s workgroups referral network, and supporting individual and organizational participation in the Connect2Thrive efforts. As part of the Community Information Exchange (CIE) efforts at United Way, this role will continue to support the development of the Connect2Thrive CIE and support the alignment and integration of community partners, client services workstreams, and data to ensure a holistic, proactive, and person-centered approach to our system design and programs. We are looking for a strategic-minded and independent leader with an adaptive style who is passionate about developing creative solutions to address challenges in the systems of care. The Connect2Thrive Partner Integration Sr Manager is an excellent communicator, and has proven experience working closely and collaboratively with diverse styles both internally and externally.

Requirements

  • Bachelor’s Degree or higher in Public Health, Social Welfare or related field is preferred.
  • At least 5 years’ experience with social or health-related programs in the Central Texas area.
  • Experience in managing multi-sector and mult-organization projects that include incorporating community voice is preferred.
  • Ability to build and maintain collaborative relationships.
  • High project management and attention-to-detail skills while managing priorities and deadlines effectively.
  • Experience in evaluation, data analysis and project improvement.
  • Understanding of social determinants of health required.

Nice To Haves

  • Bilingual in Spanish preferred.

Responsibilities

  • Collaborate with the Senior Director of the development of Connect2Thrive, including partner and program integration, technical infrastructure development, and community engagement efforts.
  • Foster and sustain strong partnerships to assess needs, identify alignment opportunities, and secure support for advancing care coordination and planning initiatives.
  • Advances and administers Connect2Thrive workgroups to support development of uses, cases, standards, and workflows that advance care planning.
  • Center Connect2Thrive participants in the design, evaluation, standards, and implementation of use cases by facilitating community member leadership and creating opportunities for meaningful participant input throughout Connect2Thrive governance.
  • Leads the implementation and continuous improvement of collaborative governance engagements and activities in support of network and administrative capabilities.
  • Leads the advancement of the Connect2Thrive Central Texas Referral Network to support community care planning, shared definitions, best practices, and evaluation of effectiveness in collaboration with the Community Resource & Engagement Team.
  • In partnership with the Senior Director, provide support to Community Invest Grant awardees to ensure successful participation in UW programming and initiatives.
  • Draft communications about Connect2Thrive, including recruitment materials and reports to United Way and Connect2Thrive audiences.
  • Ensure timely completion of grant deliverables, draft grant reports, and contribute to grant applications for programming.
  • Provide thought leadership and vision for the Connect2Thrive and Community Health and Resource Navigation department.
  • Represent United Way at external events, meetings, conferences, and committees and participate in UW special events to support our fundraising efforts.
