About The Position

The CONNECT/Clinic Without Walls program, launched in partnership with DOHMH in January 2022, provides rapid access to services through a flexible, holistic, community-based approach to mental health care. CONNECT improves quality of life by increasing access, engagement, and retention in services while reducing mental health emergencies, inpatient admissions, and mobile crisis utilization. The CONNECT Peer supports individuals with substance use and/or mental health challenges in their recovery journey. Drawing from lived experience, the Peer helps clients set recovery goals, access community resources, strengthen daily living skills, and engage meaningfully in treatment. This role includes both onsite and community-based work and collaboration with the CONNECT and clinic treatment teams.

Requirements

  • High School Diploma or GED required
  • OMH Certified Peer preferred (provisional certification accepted)
  • Personal lived experience of substance uses and/or mental health recovery with sustained abstinence required
  • Prior peer work experience required (internship alone not sufficient)
  • WRAP training preferred
  • Mindfulness training preferred
  • Bilingual English/Spanish preferred
  • CPR and First Aid certification a plus
  • Basic computer proficiency
  • Strong communication skills, patience, and empathy
  • Ability to walk throughout the clinic and community settings, including stairs
  • Ability to carry materials (folders, tablet, notebook, phone) up to 20 pounds for several hours daily

Responsibilities

  • Support individuals and groups in developing and advancing personalized recovery goals
  • Collaborate with CONNECT and clinic staff to integrate recovery-oriented practices into treatment
  • Facilitate one-on-one and group sessions focused on goal setting, recovery planning, and skill development
  • Assist clients in coordinating with case managers and treatment teams to achieve self-directed recovery
  • Provide onsite and community-based interventions
  • Support the development and maintenance of mutual/self-help groups
  • Teach and model problem-solving strategies in individual and group settings
  • Share lived recovery experience to promote hope and resilience
  • Facilitate discussions that address negative self-talk, fear, and daily living challenges
  • Participate in community outreach and presentations in partnership with the Community Liaison
