About The Position

The Complex Care Consultant (OT) position at Alliance Health is designed to support members transitioning from Adult Care Homes (ACH) to community-based living. This role involves providing comprehensive care management, conducting assessments, and collaborating with various healthcare providers to ensure members receive the necessary services for a successful transition. The position allows for a hybrid work schedule, requiring onsite presence one day a week, and involves significant travel within the communities served by Alliance Health.

Requirements

  • Master's degree in Occupational Therapy.
  • At least two years of full-time, post-graduate experience.
  • Active, valid NC Occupational Therapy license.
  • Demonstrated knowledge of assessment and treatment of mental health and substance abuse.
  • Knowledge of legal, waiver, and accreditation standards.
  • Ability to independently organize multiple tasks and manage a caseload under pressure.
  • Exceptional interpersonal and communication skills.
  • Proficiency in Microsoft Office products.

Responsibilities

  • Support members transitioning from institutional care settings to community-based care.
  • Provide subject matter expertise regarding members' physical health to support care management.
  • Collaborate with Alliance staff, behavioral health providers, and community partners to support members.
  • Utilize person-centered planning and motivational interviewing to gather information.
  • Perform individual assessments for members with complex health conditions.
  • Educate members and guardians about self-care strategies and available treatment options.
  • Collaborate with care team members to ensure care plans reflect clinical needs and life goals.
  • Update assessments and plans of care as needed.
  • Provide medication reconciliation and education.
  • Develop and update plans of care based on assessments and interventions needed.
  • Identify gaps in services and ensure appropriate care levels are met.
  • Evaluate members' homes for necessary improvements or equipment.
  • Provide support for 90 days post-transition to reduce crisis service utilization.
  • Escalate high-risk members to supervisors as needed.
  • Ensure clinical documentation meets state and Medicaid requirements.
  • Utilize data to analyze member needs and guide staff training.
