About The Position

Under general supervision, the position involves prepping, scanning, and validating all types of paper documents into an optical scanning system to ensure chart completion. The role includes scanning and validating loose reports sent to the department, analyzing medical records for deficiencies in chart documentation, and managing the release of information for continuity of care requests. The individual must ensure compliance with HIPAA guidelines to mitigate the hospital’s liability in releasing records or protected health information. Additional responsibilities include processing requests for government audits and specialized chart reviews, monitoring incomplete charts, and conducting regulatory documentation audits. The position also requires adherence to guidelines set by JC and Conditions of Participation, as well as federal and state laws. Other duties may be assigned as needed.

Requirements

  • High School completion / GED.
  • 1 - 3 years of job-related experience in a medical office.
  • Knowledge of Medical Terminology or completion of a medical terminology course within 3 months of hire.

Nice To Haves

  • Experience in Health Information or related medical field.
  • Knowledge of Word & Excel.

Responsibilities

  • Accurately prep, scan, and validate all chart types according to procedure guidelines.
  • Analyze charts for deficiencies in provider documentation across various chart types.
  • Understand and comply with HIPAA and Compliance Program.
  • Perform Release of Information processes for continuity of care requests.
  • Log all requests into the electronic medical record system.
  • Process requests for attorneys, insurance companies, patients, and government audits.
  • Pick up discharges as required.
  • Assist with weekly monitoring and provider notification process based on Voluntary Relinquishment policy.
  • Assist physicians and clinicians with chart completion.
  • Provide timely and courteous assistance to walk-in customers.
  • Monitor Epic work queues as assigned.
  • Complete Regulatory Documentation Audits.
  • Participate in validation testing during Epic upgrades as needed.
  • Maintain productivity and quality standards.
  • Understand the Premier Medical Staff Bylaws/Rules & Regulations.
