About The Position

Open Sky’s Community Supports Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting. The Community Support Navigator engages with members as they enter the treatment system and support them in gaining access to treatment services and community resources. They provide case management and direct outreach support to their assigned caseloads, which consist of a combination of members enrolled in Community Support Program, Recovery Support Navigation and/or Community Support Program for People Experiencing Chronic Homelessness. Other Key Responsibilities: Develop a Support Plan with the member that guides services in a way that is consistent and supportive of other existing comprehensive care plans. Complete all Assessments and Support Plans according to prescribed timelines and specifications. Act as a liaison between members, community agencies, and families to coordinate treatment, including employment and education entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others. Work collaboratively with medical and behavioral health providers to obtain any active comprehensive care plans that the member may have. Provide temporary assistance with transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources. Participate in an On-Call rotation.

Requirements

  • High School Diploma, GED or equivalent, required.
  • Minimum 2 years experience working with underserved populations such as adults experiencing homelessness, adults with justice involvement and those with mental illness and substance use disorders, required.
  • Valid Drivers License, reliable transportation and acceptable driving history, required.
  • Reliability, willingness to learn, and being open to new opportunities.

Nice To Haves

  • Experience with local resources, navigating health, behavioral health and/or substance abuse treatment systems strongly preferred.
