The Community Outreach Coordinator will serve as the local face of Mom’s Meals throughout New York, working directly with healthcare providers, case managers, social service agencies, and individuals to promote and facilitate the growth and access to Mom’s Meals. This role is responsible for driving awareness, building relationships, and supporting program enrollment to help ensure nutritious meals reach those who need them most. This position reports to the Manager of Healthcare Partnerships. The successful candidate will drive an increase in HDM referrals from a variety of organizations throughout New York. This position will be based in New York, ideally located in Albany, Schenectady, or Troy (with consideration given to nearby regional hubs such as Binghamton or Saratoga Springs). This role will support Social Care Network (SCN) regions including the Hudson Valley, Capital Region, North Country, Central New York, and Southern Tier. The position will travel regionally and statewide up to 75% and is eligible for a company car.
