Responsibilities

  • Make client assessments to determine individual strengths and needs in various areas.
  • Conduct in-home visits to monitor the home environment for safety and health issues.
  • Provide information, education, and referral assistance to clients and families.
  • Assist clients in obtaining services for outstanding warrants, housing, and transportation.
  • Develop and maintain productive relationships with community referral resources.
  • Provide training in daily living skills such as housekeeping and cooking.
  • Maintain regular contact with clients to keep them engaged in treatment.
  • Provide crisis intervention services as needed.
  • Attend court proceedings and provide testimony as a member of the treatment team.
  • Participate on Family Court and community task forces and special committees as requested.
