About The Position

The Community Support Specialist is responsible for providing comprehensive care coordination and support to Medi-Cal Members enrolled in Community Supports (CS). This role serves as the primary point of contact for Non-TAY Members and their support networks, helping them access and navigate non-medical services that address social drivers of health. The Community Support Specialist ensures Members receive person-centered assistance related to housing stability, food and nutrition supports, home- and community-based services, and other CS offerings. This position emphasizes culturally responsive, community-based engagement and plays a critical role in connecting Members to resources that promote safety, independence, and improved quality of life. The Community Support Specialist ensures service delivery complies with all Department of Health Care Services (DHCS) and Managed Care Plan (MCP) Community Supports requirements.

Requirements

  • High school diploma or GED required.
  • Minimum of two (2) years of experience in case management, system navigation, social services, or related work.
  • Proficiency with computer-based technology including Electronic Health Records.
  • Ability to engage and build trust with diverse and underserved populations.
  • Pass agency-paid criminal justice screening including fingerprints.
  • Pass agency-paid TB (Tuberculosis) screening.
  • Pass agency-paid health screening and/or drug testing, if required.
  • Valid California driver’s license and proof of auto insurance.
  • Pass agency-paid MVR clearance with no more than 2 points—if you are under age 25, our insurance carrier does not allow any points.

Nice To Haves

  • Bachelor’s degree in social work, psychology, public health, or related field preferred.
  • Prior experience as a Community Health Worker, Navigator, or similar field-based role preferred.
  • Knowledge of CalAIM Community Supports preferred.
  • Training or demonstrated knowledge of Motivational Interviewing, Harm Reduction, and Trauma-Informed Care preferred.

Responsibilities

  • Use comprehensive assessments to identify needs related to housing stability, food security, environmental modifications, tenancy supports, personal care, respite, and other CS services.
  • Collaborate with the Member, family, caregivers, and care team to develop person-centered support plans.
  • Reassess housing support plans at least every six (6) months or upon significant change in needs.
  • Serve as the primary point of contact for the Member, family, Authorized Representative, caregiver(s), and multidisciplinary team.
  • Coordinate communication among community partners, housing providers, health plans, and service agencies to ensure seamless support.
  • Facilitate warm handoffs across service settings, including hospital discharges and transitions to community-based supports.
  • Consult with the CS Clinical Consultant (licensed clinician) for guidance and support when behavioral or clinical considerations arise.
  • Provide Community Supports services as defined by DHCS, including outreach and engagement, assessment, linkages to non-medical supports, housing navigation, tenancy sustaining services, and coordination with social service providers.
  • Educate Members and families about available supports, rights, and self-advocacy.
  • Deliver services in the Member’s preferred setting (home, community, field, office, or telehealth).
  • Maintain accurate, timely, and complete documentation in accordance with agency, MCP, and DHCS Community Supports requirements.
  • Ensure all assessments, support plans, and service encounters meet CS quality, timeliness, and data submission standards.
  • Provide services in alignment with DHCS Culturally and Linguistically Appropriate Services (CLAS) standards.
  • Demonstrate respect for diverse cultural values, beliefs, and communication needs.
  • Participate in regular supervision, case reviews, and multidisciplinary meetings.
  • Support and guide care team members to ensure coordinated delivery of CS services.
  • Attend required trainings and maintain updated knowledge of CalAIM Community Supports and best practices in care coordination.
  • Represent the agency in a professional and competent manner.
  • Advocate for the best interests of the agency and clients served.
  • Establish effective working relationships with the public, co-workers, clients, and individuals from diverse backgrounds.
  • Promote and support a culturally and linguistically diverse workforce reflective of our service community.
  • Maintain confidentiality consistent with legal and agency standards.
  • Participate in required meetings, trainings, conferences, and serve as an agency representative at designated community meetings.
  • Demonstrate promptness, preparation, and responsibility in scheduling.
  • Perform duties with a positive attitude and collaborative spirit.
  • Communicate effectively both orally and in writing.
  • Follow safety procedures to ensure a safe working environment.
  • Participate in or support agency fundraising activities as assigned.
  • Prepare reports, documentation, statistical data, and job performance materials in a timely manner.
