Requirements

  • Strong interpersonal and motivational skills.

  • Flexibility to support clients in community and home settings.
  • Strong organizational and documentation skills.
  • Bachelor’s degree preferred. In lieu of Bachelor’s degree, Associates degree or College coursework equivalent (2 years) and 2 years work exp in behavioral health field OR high school diploma and minimum of 4 years of experience in the behavioral health field required.
  • Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
  • Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).

Nice To Haves

  • Experience with psychiatric rehabilitation, recovery planning, or related services preferred.
  • RSS certification, mental health certification or related and acceptable training preferred.
  • Knowledge of Psychiatric Rehabilitation is desirable.

Responsibilities

  • Provide individual and group sessions, supporting clients with daily living skills, recovery tools, self-advocacy, and health and wellness.
  • Conduct functional assessments and develop personalized, stage-wise recovery plans that are reviewed and updated regularly.
  • Monitor and document client progress while maintaining accurate clinical records.
  • Offer crisis intervention and outreach as needed.
  • Assist clients in connecting to medical, psychiatric, educational, vocational, and community resources.
  • Provide education and support to families and natural supports to help clients succeed.
  • Collaborate with the program team to ensure high-quality, person-centered care.
  • Provide limited transportation as clients learn independent skills (use of personal vehicle required).
