About The Position

The position involves teaching, modeling, and coaching activities of daily living (ADL) such as personal hygiene, housekeeping, meal preparation, and money management. The role includes monitoring performance of ADL skills and providing therapeutic feedback. Additionally, the position requires teaching psychoeducation classes on recovery, planning and facilitating community-based activities, and contributing to a safe and therapeutic environment. The individual will serve as an ambassador for SCS and its clients, collaborating with community systems and providing transportation as needed. Personal care, medication administration, and documentation of services are also key responsibilities. The job requires maintaining confidentiality and participating in required meetings and trainings.

Requirements

  • High school diploma or equivalent required
  • Completion of all required trainings within 6 months of hire date
  • One year full time experience working with individuals who receive services for treatment of mental health or co-occurring disorder
  • Formal documentation by supervisor of ability to function independently as direct support professional
  • Ability to provide caring, nurturing attention and care to individuals with disabilities
  • Good interpersonal skills
  • Ability to communicate with verbal and nonverbal individuals

Responsibilities

  • Teaches, models, and coaches activities of daily living (ADL)
  • Monitors performance of ADL skills and provides therapeutic feedback
  • Teaches psychoeducation classes on recovery
  • Plans and facilitates community-based activities
  • Contributes to a safe and therapeutic environment
  • Serves as an ambassador of SCS and its clients
  • Collaborates with community systems on behalf of individuals served
  • Provides transportation ensuring adherence to safety practices
  • Provides personal care as needed
  • Administers First Aid and CPR as needed
  • Supervises self-administered medication
  • Identifies clients’ food and/or medication allergies
  • Communicates relevant information with other staff members
  • May take responsibility for specific assignments within the program
  • May provide limited on-the-job training to Community Support Providers
  • Documents services and care provided
  • Maintains confidentiality of protected health information
  • Attends and participates in required staff and service planning meetings
  • Maintains and enhances skills for working with clients
