About The Position

SHIELDS’ mission is to develop, deliver and evaluate culturally sensitive, comprehensive service models that empower and advocate for high-risk families in South Los Angeles. The Community Support Specialist (CSS) will be responsible for administering Housing Community Support Services to Medi-Cal beneficiaries confronting homelessness and complex health and social needs in South Los Angeles.

Requirements

  • Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field.
  • High School Diploma or GED
  • Present ideas, information, and viewpoints clearly, both verbally and in writing.
  • Efficiently use the personal computer to include a variety or word processing software.
  • Utilize analysis, experience, and judgment to make decisions.
  • Demonstrate commitment to team objectives and SHIELDS For Families philosophies.
  • Adapt to changing needs by acquiring new skills and knowledge.
  • A valid California driver's license is required for this position, as driving is an essential function of the role and is required for field work, client crisis response and emergencies.
  • A current automobile insurance is required.

Responsibilities

  • Conducting intake and personalized needs assessments for all clients, collaborating to craft an Individualized Service Plan (ISP) targeting housing goals, income maintenance/increase, and other personal objectives outlined by the client. This encompasses addressing medical, mental health, substance use, financial resources, vocational, and social support needs.
  • Facilitating referrals, connections, information dissemination, and providing support to resources aiding clients in achieving their ISP objectives.
  • Completing assessments tailored to the program (e.g., VI-SPDAT).
  • Assessing strengths and obstacles in achieving short-term and long-term goals, including conducting a 90-day review with the client post-initial intake.
  • Adhering to documentation standards mandated by the Program and SHIELDS policies, which entails timely completion of case file notes and maintenance of a comprehensive, accurate, and up-to-date client file containing all required documents.
  • Upholding client file confidentiality in accordance with HIPAA and all relevant guidelines.
