About The Position

The Per Diem Community Support Services Counselor position involves providing Individualized Medication Recovery (IMR) and crisis intervention services to clients. The counselor will assist clients with daily living skills, including food shopping and laundry, and will implement prescribed treatment plans while helping to develop Individual Recovery Plans (IRPs). The role requires the ability to work independently and travel to consumers' residences as needed. Additionally, the counselor will monitor medication in the consumers' homes and complete necessary documentation.

Requirements

• BSW in social work, BA/BS in psychology.
• One year of experience in direct care preferred.
• Valid New Jersey driver's license.

Responsibilities

• Provides IMR and crisis intervention services.
• Assists with food shopping, laundry and daily living skills.
• Implements prescribed treatment plan and assists in the development of IRPs.
• Must be able to work independently and travel to consumers' residencies as required.
• Monitors medication in the consumers home.
• Completes documentation as required.
