About The Position

The Per Diem Community Support Services Counselor is responsible for providing Individualized Medication Recovery (IMR) and crisis intervention services to clients. This role involves assisting clients with daily living skills, including food shopping and laundry, and implementing prescribed treatment plans while aiding in the development of Individual Recovery Plans (IRPs). The counselor must be able to work independently and travel to consumers' residences as required. Additionally, the position includes monitoring medication in the consumers' homes and completing necessary documentation as required.

Requirements

• BSW in social work, BA/BS in psychology
• One year of experience in direct care preferred
• Valid New Jersey driver's license

Responsibilities

• Provides IMR and crisis intervention services.
• Assists with food shopping, laundry and daily living skills.
• Implements prescribed treatment plan and assists in the development of IRPs.
• Must be able to work independently and travel to consumers' residencies as required.
• Monitors medication in the consumers home.
• Completes documentation as required.
