The Per Diem Community Support Counselor (CHAMP) is responsible for the implementation of Individual Recovery Plans (IRPs), performing Illness Management and Recovery (IMR) services, and providing continuous input to all changes of consumer IRPs. This role includes providing on-site case management services to an assigned caseload and supervision to consumers. The counselor will facilitate a variety of social, recreational, and therapeutic groups, encouraging consumers to participate. Additionally, crisis intervention skills will be utilized as needed, and all consumer concerns must be reported to program managers. It is essential to follow the agency's documentation procedures correctly when updating consumers' electronic charts.
