One Community. One Mission. One California Job Summary and Responsibilities Position Summary The Nurse Practitioner (NP) will provide clinical support to our Street Medicine team. NP will provide direct medical care to unsheltered, homeless individuals with comorbid physical, mental health, and substance use disorders in a wide variety of settings including encampments and on the street. The NP will work as part of a multidisciplinary team consisting of behavioral health, community health workers, medical assistants, and a nurse to develop a treatment plan and provide ongoing care to a panel of high-risk homeless patients. p articipates in strategic planning within overall needs in function of growth and clinical enterprise of Street Medicine, f acilitates relationships with other practitioners and learners, p rovides ongoing education to staff, learners and community on health and homelessness. Participates in the development of new protocols for street medicine along with the director of clinic operations and medical director, p articipates in the development of standards for clinical application of new or established guidelines for patients, p articipates in local and state street medicine organizations (national and international optional), p articipates in the development of quality standards for clinical operation in Inpatient and Outpatient settings along with the director of clinic operations and medical director. Oversees quality of patient care with input from the clinical program leader and medical director, e nsures accurate tracking and dispensing of pharmaceuticals, r esponsible for ensuring timely and accurate documentation, a ssists in the development of Policies and Procedures with input from directors of clinic operations and medical director. Perform other duties as assigned or requested, and u nder the general supervision of a primary care physician to perform primary medical, obstetrical and pediatric care to adult, geriatric, adolescent, and pediatric patients, p erforms routine medical examinations of adult, geriatric, adolescent, and pediatric patients. Diagnose common medical conditions, educate and counsel patients, regarding their physical health in a family practice clinic, and perform related clinical duties. Please note that this position is for our Street Medicine Program, and t his position is a grant-funded position and may be eliminated if funds are no longer available.
